For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Summary

Etrasimod can cause a transient fall in heart rate and AV conduction delay when started. Diltiazem also slows sinus rate and AV nodal conduction, so the bradycardic and conduction effects may be additive. The effect of adding a rate-lowering calcium channel blocker is not well characterized.

What this interaction means in plain words

Diltiazem and Etrasimod can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Diltiazem belongs to Cardiovascular; Etrasimod belongs to Gastrointestinal Tract.

Evidence level: Low

Why it happens

Additive pharmacodynamic depression of sinus node function and AV conduction.

What to do

Review ECG and baseline heart rate before starting etrasimod in patients taking diltiazem, and monitor heart rate and PR interval after initiation. Seek cardiology advice if there is pre-existing conduction disease or bradycardia.

What to ask your pharmacist
