Summary

Opioids given with MAOIs can cause unpredictable reactions, including serotonin-like toxicity, profound sedation, respiratory depression, and swings in blood pressure. Dipipanone is related to methadone and pethidine, so similar risks are expected.

What this interaction means in plain words

Dipipanone and Phenelzine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Low

Why it happens

Probably serotonergic excess and potentiated CNS depression in the presence of MAO inhibition; not fully defined.

What to do

Avoid dipipanone in patients taking an MAOI or within 14 days of stopping it. Choose an alternative analgesic and monitor respiration and blood pressure if an opioid is unavoidable.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
