Summary
Dobutamine is a catecholamine that is mainly metabolized by COMT and not significantly by MAO, so the interaction is less pronounced than with indirect sympathomimetics. Transdermal selegiline at higher doses can lose MAO-B selectivity, however, and may enhance pressor responses to sympathomimetics. A hypertensive reaction is possible but not well documented for dobutamine.
What this interaction means in plain words
Dobutamine and Selegiline transdermal can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Possible potentiation of sympathomimetic pressor effects through MAO inhibition by non-selective selegiline.
What to do
Start dobutamine at a low dose and titrate carefully while monitoring blood pressure and heart rate. Stop the infusion or reduce the rate if a hypertensive response occurs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
