Summary
Antiretrovirals can inhibit lentiviral vector transduction of the patient's collected stem cells, which may compromise manufacture and engraftment of the gene therapy product. Antiretrovirals must therefore be stopped before stem cell mobilization and apheresis.
What this interaction means in plain words
Elivaldogene autotemcel and Stavudine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Antiretroviral reverse transcriptase inhibitors can interfere with lentiviral vector transduction during manufacturing of the gene-modified cells.
What to do
Stop stavudine at least 1 month before mobilization and keep it off until all apheresis cycles are completed. Plan alternative HIV management with the infectious disease team.
