Summary
Tenofovir is eliminated by the kidney and is nephrotoxic, and NSAIDs can reduce renal perfusion. Coadministration has been associated with acute kidney injury, and cobicistat further raises serum creatinine by inhibiting tubular secretion, which can mask changes.
What this interaction means in plain words
Elvitegravir / cobicistat / emtricitabine / tenofovir df and Meclofenamic acid can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive renal injury from NSAID-induced hemodynamic changes and tenofovir proximal tubular toxicity, with possible competition for renal elimination.
What to do
Use an alternative analgesic such as paracetamol; if an NSAID is necessary, use the lowest dose briefly and monitor creatinine, estimated clearance and phosphate.
