Summary
Thyroid hormone replacement at doses that produce a hyperthyroid state can raise blood glucose and increase glucose requirements in people with diabetes. Glycemic control on empagliflozin-metformin may therefore worsen after liotrix is started or its dose is increased. Glucose may fall again when thyroid hormone is reduced or stopped.
What this interaction means in plain words
Empagliflozin-metformin and Liotrix can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Thyroid hormones increase hepatic glucose output and glucose absorption and oppose the glucose-lowering effect of antidiabetic drugs (pharmacodynamic antagonism).
What to do
Monitor blood glucose and HbA1c when liotrix is started or its dose changed, and adjust antidiabetic therapy as needed. Keep thyroid replacement at the lowest dose that normalizes TSH.
