Summary

Protamine is an antidote that binds heparins and neutralizes their anticoagulant activity, rather than adding to it. It partially reverses enoxaparin, neutralizing the anti-IIa effect fully but the anti-Xa effect only partly.

What this interaction means in plain words

Enoxaparin and Protamine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

Protamine, a cationic protein, forms inactive complexes with the anionic enoxaparin, partially neutralizing its anticoagulant effect.

What to do

Use protamine intentionally for enoxaparin overdose or life-threatening bleeding, dosed according to the enoxaparin amount and time since last dose. Monitor for hypersensitivity and hypotension during infusion and for residual anti-Xa activity.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
