Summary
Reteplase is a thrombolytic that dissolves fibrin clots, and combining it with enoxaparin increases the risk of serious bleeding, including intracranial hemorrhage. In some settings such as acute myocardial infarction they are used together deliberately, but the bleeding risk remains substantial.
What this interaction means in plain words
Enoxaparin and Reteplase can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive hemostatic impairment: fibrinolysis from reteplase plus anticoagulation from enoxaparin's antithrombin-mediated factor Xa inhibition.
What to do
Use only when there is a clear indication and follow the protocol-specific enoxaparin dosing alongside lytic therapy. Monitor closely for bleeding, including neurological changes, and avoid invasive procedures and intramuscular injections.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
