Summary

Heparins can suppress aldosterone secretion, and valsartan blocks angiotensin II effects on aldosterone, so together they can raise serum potassium. The risk is greater with renal impairment, diabetes or prolonged heparin use.

What this interaction means in plain words

Enoxaparin and Sacubitril / valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive suppression of aldosterone-mediated potassium excretion, resulting in hyperkalemia.

What to do

Monitor serum potassium and renal function, especially in high-risk patients or with prolonged therapy. Treat hyperkalemia and reconsider therapy if levels rise.
