Summary
Racemic epinephrine is a direct sympathomimetic whose effects are intensified in patients on MAO inhibitors. Severe hypertension, headache and arrhythmias can follow, even with inhaled or topical use.
What this interaction means in plain words
Epinephrine racemic and Isocarboxazid are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Why it happens
MAO inhibition increases availability of catecholamines and sensitizes adrenergic receptors, giving additive pressor and cardiac effects.
What to do
Avoid use during isocarboxazid therapy and for 14 days after stopping it. Use non-adrenergic alternatives for airway symptoms.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
