Summary
Racemic epinephrine, used mainly by inhalation, has little systemic exposure at usual doses, and the QT-based concern is theoretical. Systemic sympathomimetic effects could in principle add to the small QT effect of paroxetine, and SSRIs may modestly heighten pressor responses.
What this interaction means in plain words
Epinephrine racemic and Paroxetine have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Why it happens
Theoretical additive effects on cardiac repolarization and adrenergic response; not firmly established.
What to do
No routine change is needed with inhaled use. Monitor heart rate, blood pressure and rhythm if larger or repeated doses are given in patients with heart disease.
