Summary

Telavancin can prolong the QT interval, and epinephrine has sympathomimetic effects that may provoke arrhythmias. Systemic use together may add to the risk of ventricular arrhythmia, especially with hypokalemia. Inhaled racemic epinephrine has low systemic exposure, so the real-world risk is generally small.

What this interaction means in plain words

Epinephrine racemic and Telavancin have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Low

Why it happens

Additive proarrhythmic effects: telavancin prolongs ventricular repolarization while adrenergic stimulation increases arrhythmia susceptibility.

What to do

Obtain an ECG if systemic or high-dose epinephrine is needed and correct potassium and magnesium abnormalities. No action is usually needed for inhaled racemic epinephrine in patients without other QT risk factors.
