Summary

Heparin can suppress aldosterone secretion, and combined with an angiotensin receptor blocker this may produce hyperkalemia. The risk is higher in patients with renal impairment, diabetes or prolonged heparin therapy. The thiazide component partially counteracts the effect.

What this interaction means in plain words

Eprosartan-hydrochlorothiazide and Heparin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Eprosartan-hydrochlorothiazide belongs to Cardiovascular; Heparin belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Heparin inhibits adrenal aldosterone synthesis and ARBs reduce aldosterone action, additively decreasing renal potassium excretion.

What to do

Check serum potassium at baseline and periodically during heparin treatment lasting more than a few days, especially in patients with diabetes or kidney disease. Treat hyperkalemia if it emerges.
