Summary

NSAIDs such as parecoxib (a COX-2 inhibitor prodrug) can reduce renal prostaglandin synthesis, which blunts the antihypertensive effect of ARBs and thiazides. Together with a diuretic and an ARB this raises the risk of acute kidney injury, particularly in dehydrated or elderly patients, and may cause hyperkalemia.

What this interaction means in plain words

Eprosartan-hydrochlorothiazide and Parecoxib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

Inhibition of renal vasodilatory prostaglandins reduces glomerular perfusion and sodium excretion, adding to the effects of angiotensin blockade and diuretic volume depletion.

What to do

Maintain hydration and monitor blood pressure, serum creatinine and potassium after starting parecoxib. Use the lowest effective dose for the shortest duration.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
