Summary
Salsalate is a salicylate NSAID that can reduce the antihypertensive effect of ARBs and thiazides. With a diuretic and ARB, NSAIDs increase the risk of acute kidney injury, especially in elderly or volume-depleted patients, and may raise potassium.
What this interaction means in plain words
Eprosartan-hydrochlorothiazide and Salsalate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Inhibition of vasodilatory renal prostaglandins reduces renal blood flow and natriuresis, compounding angiotensin blockade and diuretic-induced volume depletion.
What to do
Monitor blood pressure, serum creatinine and potassium, ensure adequate hydration, and use the lowest effective salsalate dose for the shortest time.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
