Summary
Eprosartan blocks the AT1 receptor, through which synthetic angiotensin II raises blood pressure, so the pressor response may be reduced. This may matter when angiotensin II is used for vasodilatory shock in a patient taking an ARB, and hydrochlorothiazide-related volume depletion may add to hypotension.
What this interaction means in plain words
Eprosartan-hydrochlorothiazide and Synthetic human angiotensin ii can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Competitive AT1 receptor antagonism by eprosartan opposes the vasopressor action of exogenous angiotensin II.
What to do
Monitor blood pressure closely and titrate angiotensin II to effect, as higher doses may be needed. Hold the ARB-thiazide combination during acute shock.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
