Summary

Erythromycin prolongs the QT interval and may cause torsades de pointes. Fingolimod causes bradycardia and may add to the effect, and erythromycin may also modestly raise fingolimod exposure through CYP inhibition.

What this interaction means in plain words

Erythromycin stearate and Fingolimod can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Additive QTc prolongation with fingolimod-induced bradycardia, plus possible CYP3A4 inhibition raising fingolimod levels.

What to do

Prefer a non-macrolide antibiotic without QT effects. If erythromycin is needed, monitor ECG, heart rate and electrolytes.
