Summary

Alcohol makes glucose control unpredictable and increases the risk of hypoglycemia with gliclazide, including delayed hypoglycemia after drinking, and may mask its warning signs. Disulfiram-like flushing reactions are occasionally reported with sulfonylureas.

What this interaction means in plain words

Ethanol and Gliclazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Ethanol inhibits hepatic gluconeogenesis and adds to sulfonylurea-stimulated insulin secretion, and may inhibit acetaldehyde metabolism in susceptible patients.

What to do

Counsel patients to avoid heavy drinking and never to drink on an empty stomach. Teach recognition of hypoglycemia and ensure food intake with any alcohol.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
