Summary
Iodinated contrast can cause acute kidney injury, which can reduce metformin elimination and precipitate lactic acidosis. Risk is greater with impaired renal function or intra-arterial administration. Ethiodized oil is used in lymphography and hepatic chemoembolization, where systemic exposure and renal risk may be present.
What this interaction means in plain words
Ethiodized oil and Glyburide-metformin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Contrast-associated kidney injury reduces renal clearance of metformin, leading to accumulation and lactic acidosis.
What to do
Hold metformin at the time of the procedure in patients with eGFR below 30, liver disease, or intra-arterial administration. Restart after 48 hours only once renal function is confirmed stable.
What to ask your pharmacist
