Overview

Severity: Moderate · Evidence level: Moderate

SNRIs impair platelet aggregation; combined with NSAIDs (including COX-2 inhibitors) increases risk of GI and other bleeding.

Mechanism of action

Pharmacodynamic: Venlafaxine depletes platelet serotonin (↓ aggregation) + NSAID-related GI mucosal injury → additive bleeding risk.

Clinical management

Avoid if high bleed risk. If needed, use lowest NSAID dose/duration; consider PPI gastroprotection; monitor for bleeding (bruising, melena, hematemesis) and CBC if prolonged use; avoid additional antithrombotics.
