Summary
Etrasimod is cleared by CYP2C8, CYP2C9 and CYP3A4. Pioglitazone is a CYP2C8 substrate and only a weak inhibitor, so the risk is mainly relevant in CYP2C9 poor metabolizers. Increased etrasimod exposure could enhance bradycardia and other adverse effects.
What this interaction means in plain words
Etrasimod and Glimepiride-pioglitazone have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Etrasimod belongs to Gastrointestinal Tract; Glimepiride-pioglitazone belongs to Endocrine.
Why it happens
Possible weak CYP2C8 inhibition by pioglitazone, reducing etrasimod clearance.
What to do
No routine adjustment is needed. Monitor heart rate after etrasimod initiation, especially in known CYP2C9 poor metabolizers.
