Summary

NSAIDs such as fenbufen can reduce the antihypertensive effect of valsartan and raise the risk of hyperkalemia. Combined use can also impair renal function, particularly in older patients, those who are volume depleted, or those with existing kidney disease.

What this interaction means in plain words

Fenbufen and Valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

NSAID inhibition of renal prostaglandin synthesis reduces renal blood flow and sodium excretion, and combined with ARB blockade of efferent tone this lowers glomerular filtration and raises potassium.

What to do

Monitor blood pressure, serum potassium and creatinine, and ensure adequate hydration. Use the lowest effective NSAID dose for the shortest time or choose an alternative analgesic.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
