Summary
Combining an ACE inhibitor with an angiotensin receptor blocker gives dual renin-angiotensin system blockade. This raises the risk of symptomatic hypotension, hyperkalemia and acute kidney injury without clear added cardiovascular benefit in most patients.
What this interaction means in plain words
Amlodipine-benazepril and Valsartan can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Amlodipine-benazepril belongs to Cardiovascular; Valsartan belongs to Cardiovascular.
Why it happens
Additive inhibition of the renin-angiotensin-aldosterone system at two sites reduces aldosterone and efferent arteriolar tone.
What to do
Avoid the combination in most patients and use one RAS blocker. If it is used for a specific indication, monitor blood pressure, serum potassium and creatinine closely.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
