Summary

Fenoldopam is a short-acting vasodilator, and beta-blockers may add to the hypotensive effect. Beta-blockade can also blunt the reflex tachycardia that normally accompanies fenoldopam, leading to greater blood pressure falls.

What this interaction means in plain words

Fenoldopam and Timolol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive pharmacodynamic hypotension: dopamine-1 mediated vasodilation plus beta-blockade, with loss of reflex tachycardia.

What to do

Monitor blood pressure continuously during fenoldopam infusion in patients on timolol, and titrate the infusion to effect. Avoid concurrent beta-blocker bolus dosing.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
