Summary
Fentanyl contributes sedation and respiratory depression on top of rocuronium paralysis. In a ventilated anesthetized patient this is expected, but in non-ventilated patients it may cause dangerous hypoventilation.
What this interaction means in plain words
Fentanyl intranasal and Rocuronium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive respiratory compromise from central opioid depression and peripheral neuromuscular blockade.
What to do
Use only with airway control and ventilation available, and monitor oxygenation and train-of-four. Reduce opioid dose in non-ventilated patients and avoid giving intranasal fentanyl while residual blockade persists.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
