Summary
Fentanyl has weak serotonergic activity, and with MAO inhibitors serotonin syndrome and excessive opioid effects have been reported. The combination can lead to agitation, hyperthermia, autonomic instability and respiratory depression.
What this interaction means in plain words
Fentanyl intranasal and Safinamide are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Why it happens
MAO inhibition combined with fentanyl's serotonergic effect increases serotonergic neurotransmission; opioid CNS depression may also be potentiated.
What to do
Avoid use with safinamide; select an opioid with less serotonergic activity if analgesia is needed and monitor closely for serotonin toxicity.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
