Summary

Fentanyl has weak serotonergic activity, and with MAO inhibitors serotonin syndrome and excessive opioid effects have been reported. Rapid transmucosal absorption can produce abrupt peaks that heighten the risk.

What this interaction means in plain words

Fentanyl transmucosal and Safinamide are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: Moderate

Why it happens

MAO inhibition combined with fentanyl's serotonergic effect increases serotonergic neurotransmission; opioid CNS depression may also be potentiated.

What to do

Avoid use with safinamide; use a non-serotonergic analgesic and watch for hyperthermia, clonus, agitation and respiratory depression if exposure occurs.

What to ask your pharmacist
