Drug interactions
ferrous fumarate + pantoprazole
For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.
Overview
Pantoprazole increases gastric pH, reducing absorption of oral ferrous salts and potentially blunting hematologic response to iron therapy.
Mechanism of action
Gastric acid suppression → decreased solubility and reduced conversion of ferric to ferrous iron → reduced intestinal absorption.
Clinical management
Avoid coadministration when feasible. If PPI is necessary, use lowest effective dose; consider taking iron with vitamin C/acidic beverage, switching to less pH-dependent or IV iron; monitor hemoglobin, ferritin, and TSAT in 2–4 weeks. Dose separation may not fully mitigate.
