Summary

Fingolimod traps lymphocytes in lymph nodes and lowers circulating lymphocyte counts. This may impair collection, expansion and persistence of CAR T cells and adds to infection risk in patients already immunocompromised by lymphodepletion and CAR T therapy.

What this interaction means in plain words

Fingolimod and Lisocabtagene maraleucel can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Fingolimod belongs to Central Nervous System; Lisocabtagene maraleucel belongs to Oncology.

Evidence level: Low

Why it happens

Fingolimod sequesters lymphocytes via S1P receptor modulation, which may reduce T-cell function and number and adds to immunosuppression.

What to do

Avoid fingolimod around leukapheresis and CAR T infusion where possible, and choose a non-immunosuppressive alternative for the underlying condition. If it cannot be stopped, monitor lymphocyte counts and closely watch for infections.
