Summary

Yellow fever vaccine is a live attenuated vaccine, and fingolimod's lymphocyte sequestration impairs immune control of vaccine virus. This risks disseminated vaccine-strain infection and a poor vaccine response.

What this interaction means in plain words

Fingolimod and Yellow fever vaccine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: Moderate

Why it happens

Fingolimod traps lymphocytes in lymph nodes, causing immunosuppression that allows replication of the live vaccine.

What to do

Avoid live vaccines during and for about two months after stopping fingolimod. Give any needed live vaccine at least 4 weeks before starting, or arrange a medical exemption for travel.
