Summary

Corticosteroids can weaken the immune response to rubella vaccine and, at immunosuppressive doses, permit excess replication of the live attenuated virus. Fludrocortisone is used at low replacement doses with mainly mineralocorticoid activity, so the practical risk is usually low.

What this interaction means in plain words

Fludrocortisone and Rubella vaccine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Corticosteroid-induced immunosuppression reduces vaccine immunogenicity and impairs control of live attenuated virus.

What to do

Review total corticosteroid exposure; replacement-dose fludrocortisone alone does not usually preclude vaccination. If immunosuppressive glucocorticoid doses are also being given, delay the vaccine according to local immunization guidance.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
