Summary
Topiramate is a weak CYP3A4 inducer and could modestly lower fludrocortisone exposure, but this has no well-documented clinical consequence. Topiramate can also cause metabolic acidosis and hypokalemia, which is a separate pharmacodynamic consideration with fludrocortisone's potassium-wasting effect.
What this interaction means in plain words
Fludrocortisone and Topiramate have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Fludrocortisone belongs to Endocrine; Topiramate belongs to Central Nervous System.
Why it happens
Possible weak CYP3A4 induction by topiramate; additive potassium loss is a separate pharmacodynamic effect.
What to do
Monitor blood pressure, serum potassium and bicarbonate when the combination is started or the topiramate dose is changed, and adjust fludrocortisone if the response is inadequate.
