What this interaction means in plain words
Fluticasone-salmeterol and Nirmatrelvir can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Fluticasone-salmeterol belongs to Respiratory; Nirmatrelvir belongs to Anti Virals.
Why it happens
Inhibition of CYP3A4-mediated metabolism of salmeterol increases its systemic concentrations.
What to do
Avoid the combination where possible, for example by using a short-acting beta-agonist or another maintenance inhaler during the antiviral course. If it cannot be avoided, monitor heart rate, rhythm and QTc and watch for tremor and hypokalemia.
