Summary

Iodinated contrast media can cause contrast-associated acute kidney injury, and foscarnet is itself significantly nephrotoxic. Giving them close together may increase the risk and severity of renal impairment, especially in dehydrated patients or those with existing kidney disease.

What this interaction means in plain words

Foscarnet and Ioversol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive nephrotoxicity through direct tubular injury and renal hemodynamic effects.

What to do

Ensure adequate hydration and use the lowest necessary contrast dose. Monitor serum creatinine and electrolytes before and for several days after contrast, and adjust foscarnet dose to renal function.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
