Summary
Heparin can suppress aldosterone secretion, and ACE inhibitors also reduce aldosterone, so together they can raise serum potassium. Hyperkalemia is more likely with prolonged heparin use, renal impairment or diabetes.
What this interaction means in plain words
Fosinopril and Heparin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fosinopril belongs to Cardiovascular; Heparin belongs to Cardiovascular.
Why it happens
Heparin inhibits adrenal aldosterone synthesis and fosinopril reduces angiotensin II-driven aldosterone release, additively impairing potassium excretion.
What to do
Check serum potassium and renal function at baseline and periodically when heparin is given for more than a few days with fosinopril. Treat hyperkalemia and review other potassium-raising drugs if it develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?

Frequently asked questions
Can I take Fosinopril with Heparin?
MediHelp rates the interaction as moderate. Fosinopril and Heparin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken. Heparin can suppress aldosterone secretion, and ACE inhibitors also reduce aldosterone, so together they can raise serum potassium. Hyperkalemia is more likely with prolonged heparin use, renal impairment or diabetes.
How serious is the interaction between Fosinopril and Heparin?
MediHelp rates it moderate, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Check serum potassium and renal function at baseline and periodically when heparin is given for more than a few days with fosinopril. Treat hyperkalemia and review other potassium-raising drugs if it develops.
