Summary
ACE inhibitors can enhance insulin sensitivity and occasionally lead to hypoglycemia in patients on antidiabetic therapy. They may also cause renal function decline, which can raise metformin levels and the risk of lactic acidosis. Sitagliptin has a small added risk of angioedema with ACE inhibitors.
What this interaction means in plain words
Fosinopril and Metformin-sitagliptin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fosinopril belongs to Cardiovascular; Metformin-sitagliptin belongs to Endocrine.
Why it happens
ACE inhibition improves insulin sensitivity, and any resulting reduction in renal function reduces metformin and sitagliptin clearance.
What to do
Monitor renal function and blood glucose, particularly after starting or adjusting the dose of fosinopril, and reduce the antidiabetic dose if eGFR falls.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
