Summary

Both agents inhibit cholinesterase, so together they can cause additive cholinergic effects including bradycardia, abdominal cramps, diarrhea, increased bronchial secretions and, at excess, cholinergic crisis with muscle weakness. This is relevant in myasthenia gravis, where weakness from overdose may mimic disease worsening.

What this interaction means in plain words

Galantamine and Pyridostigmine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Galantamine belongs to Central Nervous System; Pyridostigmine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic inhibition of acetylcholinesterase.

What to do

Avoid combining unless clearly needed; monitor heart rate, GI symptoms and muscle strength, and adjust the pyridostigmine dose if cholinergic signs appear.

What to ask your pharmacist
