Summary
Glecaprevir and pibrentasvir inhibit OATP1B1/1B3, which can raise plasma concentrations of repaglinide, an OATP1B1 substrate. Higher repaglinide exposure increases the risk of hypoglycemia. Antiviral therapy may also improve glycemia in patients with hepatitis C, adding to this risk.
What this interaction means in plain words
Glecaprevir / pibrentasvir and Metformin-repaglinide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Inhibition of hepatic OATP1B1/1B3 uptake transporters reduces repaglinide hepatic uptake and clearance.
What to do
Monitor blood glucose closely during hepatitis C treatment and reduce the repaglinide dose if hypoglycemia occurs.
