Summary
Sulfonamide antibacterials can potentiate sulfonylurea hypoglycemia, and severe hypoglycemia has been reported. The risk is greatest in older patients and those with renal impairment.
What this interaction means in plain words
Gliclazide and Sulfadiazine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Sulfadiazine may inhibit CYP2C9 metabolism of the sulfonylurea and displace it from plasma protein, and may itself have an insulin-secretagogue effect.
What to do
Monitor blood glucose closely during co-administration and educate the patient about hypoglycemia symptoms; reduce the gliclazide dose if glucose falls.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
