Summary
Heparin can suppress aldosterone secretion, and together with valsartan this can lead to hyperkalemia. The risk is higher with prolonged heparin use, renal impairment, diabetes or other potassium-raising drugs.
What this interaction means in plain words
Heparin and Hydrochlorothiazide-valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Heparin belongs to Cardiovascular; Hydrochlorothiazide-valsartan belongs to Cardiovascular.
Why it happens
Heparin inhibits adrenal aldosterone synthesis, adding to the reduction in aldosterone effect produced by angiotensin receptor blockade; the thiazide partly offsets this.
What to do
Check serum potassium and renal function if heparin is continued beyond a few days, especially in patients with risk factors. Treat hyperkalemia and reassess therapy if potassium rises.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
