Summary
Heparin can suppress aldosterone secretion and cause hyperkalemia, even with short courses. Combined with ramipril, the risk of elevated potassium increases, especially in patients with diabetes, renal impairment or baseline high potassium.
What this interaction means in plain words
Heparin and Ramipril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Heparin belongs to Cardiovascular; Ramipril belongs to Cardiovascular.
Why it happens
Heparin inhibits adrenal aldosterone synthesis, adding to the reduced potassium excretion caused by ACE inhibition.
What to do
Monitor serum potassium and renal function during heparin therapy in patients taking ramipril, particularly if treatment lasts beyond several days.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
