Summary

Corticosteroids can weaken vascular integrity and irritate the gastric mucosa, which may increase bleeding risk in patients receiving heparin. Intramuscular or intra-articular injection in an anticoagulated patient may also cause local hematoma.

What this interaction means in plain words

Heparin and Triamcinolone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Heparin belongs to Cardiovascular; Triamcinolone belongs to Endocrine.

Evidence level: Low

Why it happens

Pharmacodynamic: corticosteroid effects on vascular fragility and gastric mucosa add to heparin-mediated anticoagulation.

What to do

Monitor for bleeding and follow aPTT or anti-Xa as appropriate for the heparin used. Consider gastroprotection with prolonged steroid exposure and avoid deep intramuscular injection when possible.

What to ask your pharmacist
