Summary

Norepinephrine is a potent vasoconstrictor and pressor that opposes the vasodilating effect of hydralazine. The blood pressure response to either drug may be unpredictable and the hypotensive effect of hydralazine may be reduced. Hydralazine may also be present in patients on vasopressor support in critical care settings.

What this interaction means in plain words

Hydralazine and Norepinephrine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Pharmacodynamic antagonism between alpha-adrenergic vasoconstriction by norepinephrine and direct arteriolar vasodilation by hydralazine.

What to do

Titrate norepinephrine to the blood pressure target with continuous or frequent blood pressure monitoring. Reassess the need for hydralazine while the patient is receiving a vasopressor.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
