Summary
Combining a vasodilator with a beta-blocker lowers blood pressure further than either alone. This combination is often used deliberately, because the beta-blocker limits the reflex tachycardia caused by hydralazine. Excessive hypotension or bradycardia is still possible, particularly at initiation.
What this interaction means in plain words
Hydralazine and Penbutolol have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Why it happens
Additive blood pressure lowering through arteriolar vasodilation plus beta-adrenergic blockade, with beta-blockade attenuating hydralazine-induced reflex tachycardia.
What to do
Monitor blood pressure and heart rate when starting or changing doses. Adjust doses to the desired blood pressure target.
What to ask your pharmacist
- Is there anything I should notice while taking both?
- Do I need to tell my doctor at the next visit?
