Summary
Indacaterol, a long-acting beta-2 agonist, can lower serum potassium, and thiazide diuretics also cause potassium loss. Together hypokalemia may be more pronounced and may predispose to arrhythmias, particularly in patients with hypoxia.
What this interaction means in plain words
Hydrochlorothiazide-telmisartan and Indacaterol inhaled can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Beta-2 mediated intracellular potassium shift is additive to thiazide-induced renal potassium loss.
What to do
Monitor serum potassium, especially at initiation and in patients with severe COPD or other risk factors. Correct hypokalemia, and note that telmisartan may partly offset the potassium loss.
What to ask your pharmacist

- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
