Summary
NSAIDs such as meclofenamate can blunt the antihypertensive effect of telmisartan and hydrochlorothiazide. The triple effect of NSAID, ARB and diuretic increases risk of acute kidney injury and hyperkalemia, especially with volume depletion or in older patients.
What this interaction means in plain words
Hydrochlorothiazide-telmisartan and Meclofenamate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
NSAID inhibition of renal prostaglandins reduces renal perfusion and sodium excretion, opposing diuretic and ARB effects and impairing glomerular filtration.
What to do
Use the lowest effective NSAID dose for the shortest time and maintain hydration. Monitor blood pressure, serum creatinine and potassium.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
