Summary

Parecoxib, an NSAID prodrug of valdecoxib, can blunt the antihypertensive effect of telmisartan and hydrochlorothiazide. Combining an NSAID with an ARB and diuretic raises the risk of acute kidney injury and hyperkalemia, particularly in volume-depleted or older patients.

What this interaction means in plain words

Hydrochlorothiazide-telmisartan and Parecoxib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

COX-2 inhibition reduces renal prostaglandins, lowering renal perfusion and sodium excretion and impairing glomerular filtration.

What to do

Maintain hydration and use the shortest effective course. Monitor blood pressure, serum creatinine and potassium.
