Summary
Pindolol is a beta-blocker that adds to the blood pressure lowering of telmisartan and hydrochlorothiazide, which is often intended but can cause hypotension or bradycardia. Beta-blockers may also blunt the rise in heart rate that normally follows excessive vasodilation or volume loss. Beta-blockade can mask hypoglycemia symptoms, and thiazides can raise glucose.
What this interaction means in plain words
Hydrochlorothiazide-telmisartan and Pindolol have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Why it happens
Additive pharmacodynamic antihypertensive effects from beta-adrenergic blockade combined with angiotensin receptor blockade and diuresis.
What to do
Monitor blood pressure and heart rate when starting or adjusting either agent, and adjust doses if symptomatic hypotension or bradycardia occurs. Monitor glucose in diabetic patients.
