Summary
Potassium acid phosphate supplies potassium, and telmisartan reduces renal potassium excretion by blocking aldosterone-mediated effects, so hyperkalemia can develop, especially in renal impairment or diabetes. Hydrochlorothiazide promotes potassium loss and may partly offset the effect, which makes the net outcome unpredictable.
What this interaction means in plain words
Hydrochlorothiazide-telmisartan and Potassium acid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Potassium load combined with ARB-induced reduced aldosterone-dependent potassium excretion, partly counterbalanced by thiazide kaliuresis.
What to do
Measure serum potassium and renal function at baseline and within the first weeks of therapy, then periodically. Reduce or stop the potassium supplement if potassium rises.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
