Summary

Procainamide and its active metabolite NAPA are cleared partly by renal tubular cationic secretion, and thiazide-type diuretics may compete or alter renal handling, which could raise levels. Hydrochlorothiazide can also lower potassium and magnesium, which increases the proarrhythmic potential of procainamide.

What this interaction means in plain words

Hydrochlorothiazide-telmisartan and Procainamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Possible competition for renal tubular secretion plus diuretic-induced electrolyte disturbances that potentiate proarrhythmia.

What to do

Monitor ECG, procainamide and NAPA levels where available, and check potassium and magnesium. Correct electrolyte abnormalities and adjust the procainamide dose if levels or QRS or QT intervals increase.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
